We all know that as we age our needs can very quickly become more complex. As providers of government-funded programs it is our responsibility to manage these changes. In home care this can be tricky as we have very limited involvement in care provision and monitoring.

 

Assessing Vulnerability & Risk

Effectively managing consumers with complex care needs is critical to the success and sustainability of your service. These consumers often present complex challenges, requiring specialised risk management and highly responsive care strategies. When these needs are not appropriately addressed, providers may face significant challenges, including non-compliance with Aged Care Quality Standards and the risk of consumer harm. Poor management of consumers with high-prevalence, high-impact risks is a common factor in instances of non-compliance. By effectively managing these needs, home care providers can safeguard their services, reduce risks, and improve outcomes for their consumers.

Navigating the Aged Care Quality Standards

Providers must align with both the current and forthcoming Aged Care Quality Standards to remain compliant. These standards emphasise consumer-directed care, high-quality service provision, and continuous improvement. Providers need to demonstrate that they have systems in place for monitoring consumer health and responding proactively to any changes in their condition.
