🌟 Are you confident your workforce is meeting Standard 5: Clinical Care when it comes to medication management under Support at Home?
With the Support at Home Program now in place and the Aged Care Act 2024 introducing heightened expectations for clinical governance, accountability, and outcomes, providers must be able to clearly demonstrate that medication support is safe, appropriate, and delivered within scope of practice. Medication management remains a high-risk area for providers and a key focus during audit and quality review activity.
To meet these requirements, providers must have:
- Trained and competent staff
- Clear and current care plans
- Robust policies and procedures
- Effective incident and risk management systems
- Accurate, defensible documentation
This training workshop provides a comprehensive, practice-focused approach to medication management, equipping both clinicians and non-clinicians with the knowledge and skills required to meet Standard 5: Clinical Care of the strengthened Aged Care Quality Standards, while operating confidently within the Support at Home framework. The session emphasises collaboration, safety, and person-centred practice to ensure medication support is delivered safely and consistently across all workforce levels.

This workshop will give you the skills and knowledge to train others in safe and compliant medication management. It will clarify exactly what your service needs to have in place under Support at Home to meet participant’s needs and regulatory expectations.
Topics we will cover:
- What is the difference between medication administration and medication support
- What is the legal framework for medication management in the community
- Standard 5 and Support at Home: What the strengthened standards and service list require for medication management, including Nursing Care and Personal Care.
- Principles of safe medication practices: Safe storage, handling, administration, and awareness of high-risk medications and common errors.
- Participant-centred medication support: Involving participants in decisions, respecting cultural preferences, and tailoring care to individual needs.
- Collaborative Practice: Communication between clinicians, non-clinicians, pharmacists, prescribers, and families.
- Recognising and Reporting: Identifying issues such as adverse reactions or errors, documentation, escalation, and RN oversight.
