Support at Home in practice: A strategic approach to cohort management

The introduction of Support at Home has fundamentally changed how providers think about their client base. Gone is the one-size-fits-all approach, replaced with the current more nuanced, needs-responsive model that requires providers to actively manage distinct client cohorts with precision and purpose.

Understanding who sits in your cohort their care goals, their funding classifications, their trajectory under the new budget and restorative support structures, is no longer just good practice. It’s a core operational and compliance requirement.

This workshop is designed for providers who want to move beyond the basics of transition and into confident, capable cohort management. We will explore what the Support at Home framework actually demands of your intake and care management processes, and how to build the internal capability to meet those demands consistently, at scale, and in line with the Act’s strengthened focus on the rights and wellbeing of older Australians.

This workshop provides a structured opportunity to examine your current approach and build greater capability across the areas that matter most: intake, care and support planning, budget management, and the use of cohort-level data to drive better decisions. 🚩 Learning Outcomes

By the end of this event, participants will be able to:

  • Assess the strengths and gaps in their current intake, assessment, and care planning processes against the requirements of the Aged Care Act 2024 and the operational demands of Support at Home delivery.
  • Apply practical frameworks for aligning workforce capacity, service design, and resource allocation to the needs of distinct client cohorts.
  • Use cohort-level data and insights to inform organisational decision-making, identify emerging trends in their client population, and respond proactively to shifting care needs.
  • Demonstrate a person-centred approach to cohort management that reflects the rights, goals, and wellbeing of older Australians
  • Develop an action plan to strengthen cohort management practice within their organisation, with clear priorities for the period ahead.
