People 65 and older have the highest risk of falling, with 30% of people older than 65 and 50% of people older than 80 falling at least once a year. Falls can cause hospitalisations, loss of independence, distress, pain, injury, and loss of confidence. Falls can also profoundly impact the family members and carers of older people.
Care managers can undertake a FRAT to identify risks and the need for further referrals and strategies
This package includes a PowerPoint presentation, a web-snap video from Lorraine Poulos RN and Jacqui Culver Nurse Practitioner, and a sample tool.
