Higher level HCP recipients and CHSP clients require skilled management of their health issues. Future reforms will require complex care managers to be skilled in assessments and effective referral mechanisms. Both clinicians (health and allied health professionals) and non-clinicians (such as care managers or support staff) can undertake some clinical assessments. Comprehensive clinical assessment is more than just filling out a few forms – it is a comprehensive process that involves gathering information, evaluating physical and mental health status, identifying needs, and developing appropriate care plans.

At LPA we believe Initial assessments by care managers should identify any health issues which then can be referred to a clinician such as a Registered or Enrolled Nurse, Physiotherapist, or Occupational Therapist for a more in-depth assessment which then allows the health professionals to gain deeper insight into individual needs.

Health issues and care needs are best identified through a comprehensive, multidisciplinary, and person-centred approach to assessment. Following effective assessments, care managers and others can clearly identify who is most vulnerable and at risk.

In this training workshop, you will come away with a tangible set of skills that give you the confidence to carry out systematic and holistic clinical assessments. This course has been revised and updated for 2024 to reflect contemporary practice, examples, and information.

By the end of this very interactive training session, you will learn:

  • Describe the requirements of clinical assessment in the relevant standards.
  • Conduct effective clinical assessment according to policy and procedures.
  • Identify which assessments can be conducted by non-clinicians.
  • Describe different types of validated assessment tools and when these should be used.
  • Practice holistic assessment: psychological, physical, functional, social, and spiritual domains.
  • Establish systems to capture key information for comprehensive care delivery.
  • Align the care plan with life goals.
  • Assess for deteriorating conditions and changes
  • Implement clinical protocols, pathways, and guidelines.
  • Apply clinical judgment, reasoning, and decision-making.
  • Describe common errors and pitfalls with clinical assessments.
  • Develop documentation for referrals and high-risk scenarios.
  • Describe strategies to partner with consumers.
  • Outline how clinical assessment relates to clinical governance.
  • Describe the scope of practice, accountability, and the ethical, legal, and professional issues.
